Grocery/Food Assistance Request

1. To be eligible for grocery/food assistance, the client must have Sickle Cell Disease or be the caregiver of a minor with Sickle Cell Disease and be a CSRA Resident (Georgia Counties:

   Burke, Columbia, Glascock, Hancock, Jefferson, Jenkins, Lincoln, McDuffie, Richmond, Taliaferro, Warren, Washington, and Wilkes. 

   South Carolina Counties:

   Aiken, Edgefield, Allendale, Barnwell, and McCormick.)

2. Client must present a valid picture ID for assistance.

3. Each client can receive a maximum of up to $200.00 per calendar year. Funds are distributed on a first come first serve basis.

4. Grocery/Food assistance will be provided in the form of a gift card to a local grocery store.

5. A list of available community resources will be provided should further assistance be required.

   Note: All assistance is based on available resources.
